Transitional support and introduction to Managed Support

When an individual moves from one healthcare setting to another, transition between systems or is just new to supported services, it is important to preserve the continuity of support with a minimum of fuss and this transitional period is managed and fulfilled by Cheshire Care.  This mainly covers individuals who before have only ever been previously supported for by family members and introduces them into the supported environment.

We adopt a process of first professionally assessing the needs and requirements of the individual before assigning to an appropriate team of staff.  These teams are highly trained and experienced in supporting individuals gradually into the service. Once assessed then an unique support plan is put into action by the team to match the individuals needs.   This introduction and overall support is gradually built up over time as individual needs increase.

Transition can be into the Day Centre, Respite, Outreach and Supported Living.

Transition takes as long as it needs to and is tailored to the individuals requirements and any specific needs and it is important to build a bond of trust between the support workers and the individual.
